NB: For University of Sheffield Orthoptic students only.

COLLECTION:
Once you have received an email confirming your order (this is also your receipt), please go to the School of Allied Health Professions, Pharmacy, Nursing and Midwifery, 362 Mushroom Lane between 10am and 3pm on a weekday to collect your card.

You will need to present your confirmation email and sign to confirm receipt of your order.

The card remains the property of Sheffield Teaching Hospitals, and must be returned when the student leaves the university at the end of their studies.
